I declare that I am aware of the rights and duties of the members of the Society and that I will act in accordance with the laws, the Statutes of the Society, the Rules and the decisions of the bodies of the Society. In accordance with the provisions of the Personal Data Protection Act - Official Consolidated Text (ZVOP-1-UPB1) (Official Journal of the Republic of Slovenia, No. 94/2007) and any subsequent amendments or additions, I agree that the FEEL DEEP Diving Association shall keep in its records and process for its own purposes my personal data as stated above, and in addition to it, my data on my activities in the Association and my other additional diving skills and specialties.

By signing this form, I authorise the Society to send me notifications related to the activities of the Society. By signing below, I authorise the FEEL DEEP Diving Association to use for its own and marketing purposes photographs taken in the context of activities and events organised by the Association, at which I am present in person.

(For persons under 18 years of age only: I, the undersigned legal guardian, declare that I consent to the minor's membership in the diving association and will inform the minor that he/she will act in accordance with the association's constitution. As his/her legal representative, I am also legally, morally and civilly liable for all his/her actions and I agree to the above provisions as well as to the provisions regarding the use of my personal data and his/her personal data.)
